APPLICATION FOR VOLUNTEERS IN PUBLIC SCHOOLS-Applications are required every 4 years/SOLICITUD PARA VOLUNTARIOS EN LAS ESCUELAS PÚBLICAS-Se requiere la solicitud cada 4 años

This application starts the process for volunteering in Public Schools.  Any and all attachments become the property of the Long Beach Unified School District.  Answer all questions completely and accurately.  All statements are subject to verification.  You may be disqualified for false statements.  The application will be kept for four years.  It is your responsibility to update your TB test, address, phone number, email and other pertinent information.  / 

Esta solicitud inicia el proceso de voluntariado en las Escuelas Públicas. Cualquier y todo adjunto se convierte en propiedad del Distrito Escolar Unificado de Long Beach. Responda a todas las preguntas de forma completa y precisa.  Todas las declaraciones están sujetas a verificación.  Usted puede ser descalificado por declaraciones falsas.  La solicitud se conservará durante cuatro años.  Es su responsabilidad actualizar su prueba de TB, domicilio, número de teléfono, correo electrónico y otra información pertinente.
